Job Description: Service Coordinator Reports to: Team Lead/Housing Coordinator

Service Coordinators are front-line members of the Yellowknife Housing First team. Under the supervision of the Team Lead, service coordinators provide and coordinate services for Housing First tenants in a flexible, compassionate and responsible manner.

 

 

Responsibilities

Responsibilities:

  • Review client intake and collaborate with clients to create a comprehensive client-centered service plan • Provide case management and advocacy to tenants, and coordinate and monitor referrals to community services and tenant participation • Maintain written and computerized records, compile reports and complete other program documentation • Assist clients to manage their finances, including preparing budgets, accessing Income Support and assisting in preparing tax and other financial documents • Educate, inform and advocate for clients regarding benefits and entitlements (e.g. disability supports, survivor benefits) • Organize and participate in social activities with tenants in the community • Assist clients to prepare for and find employment, if desired • Provide clients with education regarding their consumer rights, including grievance procedures with Yellowknife Housing First, and support to pursue any grievances • Assist in bridging any gaps between tenants and community service providers • Facilitate educational and support groups, as needed • Participate in on-call rotation • Perform related work as assigned

Qualifications

QUALIFICATIONS:

  • Previous experience working with individuals with histories of substance use, mental health issues and/or trauma; experience working with homeless individuals preferred • A degree in social work or related degree (e.g. psychology, occupational therapy) preferred • An awareness of the history of colonialism and the legacy and impact of residential schools • The ability to discuss sensitive topics effectively, and to handle disagreements in a way that leads to acceptable compromises and avoids power struggles or confrontations • The ability to work independently, to cope with crisis situations, and to work flexible hours in a changing environment • A solid understanding of harm reduction as a philosophy and practice; an understanding of social determinants of health and social justice issues • Excellent interpersonal communications skills, including conflict management skills • Excellent written and oral communications skills, as well as computer literacy in Word and Excel • Have a flexible, positive, team-based and solution-oriented approach to work • Valid NWT driver’s license required

This is a full time position working 7.5 hrs/day for 37.5 hrs/week.
